GLP-1s were initially developed to manage type 2 diabetes, but are now showing profound anti-cancer signals across three studies and more than a million patients.

Three new studies dropped. The first, in JAMA Netw. Open, tracked 841,831 women with breast cancer across 68 US hospitals. Among obese patients on GLP-1 drugs, all-cause mortality was 65% lower, with 10-year survival sitting at 96% versus 88.6% for those not on the medication. The numbers were even more stark for breast cancer patients with diabetes. Women on GLP-1 drugs had 91% lower all-cause mortality than those on insulin or metformin. That means a 10-year survival rate of 96.9% versus 76.4%, a 20% gap.

The second study, presented at ASCO 2026, tracked 12,112 patients across seven cancer types and found that GLP-1 drugs cut metastatic progression by 31% to 50%. – Lung cancer: 22% to 10% – Breast cancer: 20% to 10% – Colorectal: 22% to 13% – Liver cancer: 28% to 19%.

A separate analysis of tumor tissue found that cancers with higher GLP-1 receptor expression carry a 33% lower death risk overall, and a 45% lower risk in breast cancer. Weight loss alone does not explain these numbers, suggesting the pathway reaches the tumor directly. If the benefit were purely metabolic, any effective weight loss intervention would produce similar survival numbers. Tumors with higher GLP-1 receptor expression surviving longer suggests the drug is interacting with the cancer’s own biology. Preclinical mouse models showed tirzepatide reducing breast cancer tumor growth, with researchers now working to determine how much of that effect is direct versus driven by weight loss. GLP-1 receptors sit directly on tumor cells, so activating them could mean slower growth.

Retatrutide could go even further. As a triple agonist hitting GLP-1, GIP, and glucagon simultaneously, its phase 3 program is producing up to 28.7% average body weight loss, which is the highest ever recorded for any obesity drug. The most recent study, drawn from 110,000 women aged 45-80, also found that GLP-1 users were 30% less likely to develop breast cancer in the first place. Unlike the studies above, this one measured incidence, whether cancer developed at all, not what happened after diagnosis.

The question is no longer whether GLP-1s have anti-cancer effects, but how large those effects are, and whether the drugs we haven’t yet tested could push them even further.
